I currently collect and process the following information:

·      Medical Conditions and Medication

·      Relatives details where appropriate

 

How I get the personal information and why I have it

Most of the personal information I process is provided directly by you for one of the following reasons:

·      Enquiries

·      Booking an Assessment

·      Preliminary information prior to an assessment

·      Writing a report for a professional involved with your instruction

I use the information that you have given us in order to advise another professional instructed by you, about your personal circumstances such as current health, financial and social situation.

I may share this information with only the person named and instructed by you.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting Susan Wyatt.

(b) I have a contractual obligation.

(c) I have a legal obligation.

(d) I have a vital interest.

(e) I need it to perform a public task.

(f) I have a legitimate interest.

 

How I store your personal information

Your information is securely stored.

●     Any personal information is held on an electronic device, with password protection known only to Susan Wyatt.

 

●     Any written information during the assessment process will be destroyed by shredding after transfer to the electronic assessment document unless otherwise indicated.

 

●     The information will not be shared with any other person or company, either electronically, verbally or in writing other than the professional you have named to me.

 

●     The information will be reviewed/updated as required during the assessment process.

 

●     When the assessment process has ended this information will be disposed of by deleting from the electronic device within 14 days after sending the email with the recorded assessment attached to the Instructor. The device has a recognised electronic shredding app which is used to permanently delete data.

 

●     Any written assessment required by law to be kept will be held by Susan Wyatt in a secure locked location at 58 Shenstone Avenue, Hillmorton, Rugby. CV22 5BL for a duration of 6 years from the date of the assessment.

 

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask for copies of your personal information.

Your right to rectification - You have the right to ask to rectify personal information you think is inaccurate. You also have the right to ask to complete information you think is incomplete.

Your right to erasure - You have the right to ask to erase your personal information in certain circumstances.

Your right to restriction of processing - You have the right to ask to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that I transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
